Position Overview:
The Care Coordinator position is responsible for the DHS and CMS required activities for MSHO, MSC+, and Health Care Home Coordination. This position will independently manage a caseload, including face to face visits at facilities and in the community, completing all required documentation and communicating with the medical team. Care Coordinators provide assessment, planning, coordination of care, and evaluation of outcomes to ensure patient needs are being met.
Qualifications
:
Current Minnesota Registered Nurse or Licensed Social Worker
Two or more years of experience in geriatric and/or disability care preferred
Ability to work independently
Creative problem solving skills
Ability to work with diverse populations
Proven ability to communicate effectively with strong verbal skills
Excellent interpersonal and customer service skills
Computer proficiency
Must have a valid driver's license; local travel is required
Demonstrated compatibility with Bluestone's mission and operating philosophies
Demonstrated ability to read, write, speak, and understand the English language

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
